RESCHEDULE REQUEST FORM

Reschedule Request Fees are as follows:

$25 for the first reschedule request granted
$50 for the second
$100 for the third

If a fourth reschedule request is made, it will result in a forfeit and fine.
Or you must play the game as scheduled.

Reschedule Requests do not include weather related reschedules, field problems 
or scheduling errors.

Reschedule Requests ARE NOT guaranteed.  Whether a reschedule is granted will
depend on many factors, not the least of which is whether the opposing team is 
able to reschedule.
